How does tirzepatide work?
Tirzepatide combines two routes of action in one molecule. Via the GLP-1 receptor it is, like semaglutide, linked to suppressing appetite and slowing gastric emptying. In addition, it acts on the GIP receptor, a second incretin route that pure GLP-1 substances do not use. It is thought that precisely this combination of GLP-1 and GIP leads to a stronger metabolic response in research than a GLP-1 substance alone. What we know about this comes almost entirely from clinical studies.
Tirzepatide vs. semaglutide (Ozempic) and retatrutide
The three best-known metabolic peptides differ mainly in the number of receptors they act on. Semaglutide (Ozempic, Wegovy) is a single GLP-1 agonist: one switch. Tirzepatide adds GIP and is therefore dual: two switches. Retatrutide goes one step further and also activates the glucagon receptor, a triple agonist with three switches. In terms of mechanism, tirzepatide sits exactly between semaglutide and retatrutide. Important difference: semaglutide and tirzepatide are approved medicines, retatrutide is not (yet). Tirzepatide, Mounjaro and Zepbound
Tirzepatide is the active substance behind the brand names Mounjaro and Zepbound from manufacturer Eli Lilly. Under those names it is dispensed by prescription through the pharmacy, as a pre-filled pen.
