What is survodutide?
Survodutide (also known by the code BI 456906) is a research peptide from the group of dual incretin agonists. "Dual" means it acts on two receptors at once: the GLP-1 receptor and the glucagon receptor. It is being developed by Boehringer Ingelheim and Zealand Pharma and is mainly in view in research around obesity and fatty liver disease (MASH). Unlike most known substances in this class, survodutide is not yet approved: it is in Phase 3 trials.
How does survodutide work?
Survodutide combines two routes of action in one molecule. Via the GLP-1 receptor it is, like semaglutide, linked to suppressing appetite and slowing gastric emptying. Via the glucagon receptor it is thought to additionally increase energy expenditure and act on fat metabolism in the liver. It is precisely this glucagon component that sets survodutide apart from pure GLP-1 substances, and which draws attention in research around fatty liver. Survodutide vs. retatrutide
Survodutide and retatrutide are often mentioned side by side, because both belong to the new generation of metabolic research peptides. The difference is in the number of receptors: survodutide is a dual agonist (GLP-1 + glucagon), while retatrutide is a triple agonist (GLP-1 + glucagon + GIP). In research, survodutide stands out mainly for its focus on fatty liver (MASH); retatrutide for the strong weight effects in early studies. Survodutide vs. semaglutide and tirzepatide (Ozempic/Mounjaro)
Survodutide is sometimes called "the little brother of Ozempic" online, but it works differently. Semaglutide (Ozempic, Wegovy) is a pure GLP-1 agonist; tirzepatide (Mounjaro) acts on GLP-1 and GIP. Survodutide combines GLP-1 with glucagon, a route neither of them uses. That glucagon component is why survodutide is looked at separately in research, particularly for the liver. Research status: Phase 3, obesity and MASH
Survodutide is in Phase 3 trials, the final stage before any approval is sought. The research runs in two Phase 3 programmes: SYNCHRONIZE in obesity and LIVERAGE in MASH (metabolic dysfunction-associated steatohepatitis), a form of fatty liver. For MASH, the substance received a Breakthrough Therapy designation from the US FDA in 2024. At present, however, survodutide is not approved as a medicine anywhere. Reconstituting and storing survodutide
Survodutide is supplied as a lyophilised powder in a vial.
